A: 7 – Against 6NT, underleading the Queen of Spades bares the risk of offering the twelfth trick to your opponents.
You need to find a neutral lead, in a suit that is long and composed of small cards like your hearts.
With four small cards, against No-trump we lead the second highest so that partner does not mistake it for a fourth highest, coming from a solid suit.
