2 NT - Jacoby, forcing to game with four Spades
5 - Four or one key(s)

 Your choice:
A: J - Against a slam, when you hold an Ace there is no point in leading a singleton since your partner won't have an Ace to get the lead with and give you a ruff.

Instead you must try and set up a Club trick while you still have the Heart Ace. Your opponent are not crazy. South must have the Hearts controlled, so you won't have the Ace-King of Hearts to cash right away.

2 NT - Jacoby
5 - Two keys and the trump Queen

 Your choice:
A: 2 - The safest lead is a trump. Usually, leading a trump from Jack-x-x is dangerous but not here. Do you know why?

Your opponents have bid a grand slam, so you know they have all the keycards and the Queen of trumps. Therefore your partner has at most a small singleton and your lead won't give anything away.

Even a Club lead could be much too dangerous, possibly opening up a frozen suit.

5 - Three or zero keys

 Your choice:
A: 2 - Your partner's double is a Lightner double, asking for an unexpected lead. Against a suit contract slam, it's usually done with a side-suit void to ask for a ruff.

Here partner's void must be in Spades. You need to give him a ruff. Lead a small Spade, as a suit preference signal for a Club switch.
